Abstract: Debriefing has been used in the military for educational and operational purposes, and therapeutically for soldiers after combat. It is only in the last few years that healthcare education has adopted debriefing, which provides a way to analyze and make sense of experiences. There are many models and styles of debriefing but its effectiveness is due in large part to the facilitator and other factors.

The purpose of this presentation is to define debriefing and the goals of debriefing; and to discuss debriefing approaches and methods that lead to success in the practice.

Simulation software stimulates another 20 new jobs at Education Management Solutions in Exton

When doctors, nurses, first responders and other health care personnel are trained, it makes obvious sense for them to practice on simulators, not human beings.

From that premise, Education Management Solutions (EMS) in Exton has evolved into a fast-growing provider of software and audio-video devices used for training, evaluation and documentation by medical and nursing schools, hospitals, testing boards, allied health programs and counseling centers.

Anurag Singh founded the company as a one-man consultancy in 1994. Since then the workforce has grown exponentially, up 78 percent between 2009 and 2011. As of March, EMS's workforce was up another eight percent; now at more than 80, Singh expects to reach 100 by year's end.  To accommodate the growth, the company recently relocated to a new 37,000-square-foot headquarters.

EMS software, cameras and video recorders provide a high-tech solution for learning everything from routine tasks to "replicating a problem they may or may never see" in actual practice, Singh explains.

Moreover, he adds, "the best way to learn in any environment is to reflect on what you did right and what you did wrong. So the whole process being recorded and having experts evaluate your performance and give feedback, that is applicable in any industry."

The demand for patient safety and patient care has driven EMS's rapid growth, with sales up 153 percent in three years.  Now Singh has his sights on the international market, where he sees opportunity for continued expansion -- a s(t)imulating possibility.

Continuous Expansion Drives the Need for New Office Space

Exton, PA (April 11, 2012) – Education Management Solutions (EMS), a software developer of simulation management and audio-video solutions for training environments, was ranked #18 on Philadelphia’s Top Technology Employers 2011 list published by the Philadelphia Business Journal. In 2011, the Exton, Pa-based company increased its employee base by 23 percent compared to 2010 staffing levels, and a significant 78 percent when compared to 2009 levels. As of March 2012, EMS has already increased its employee headcount by 8 percent over 2011, and will continue to add more resources this year.

Accompanying the growth in staff, EMS enjoyed a three-year sales growth of 153%. A healthcare provider is receiving a man in a hospital emergency room who has displayed signs of a seizure-related illness. But before he can be admitted, he falls to the floor and begins experiencing a full-blown seizure. The provider at first is uncertain as to the course of action, and valuable moments are lost. But before the patient is too incapacitated, he suddenly stops and sits up. Fortunately, it turns out he is an actor in a medical simulation training facility.

The provider is called to a video screen, where the exchange and seizure is played back for debriefing and evaluation with software and hardware provided by Education Management Solutions (EMS). “This is all part of the learning process – it is a not a real-life scenario,” President and CEO Anurag Singh points out. “The best way for individuals to learn is to self-reflect and be debriefed by an instructor who facilitates the process of self-reflection instead of just pointing out issues and errors. The self-reflection process is amazing. It reinforces better understanding and learning than somebody simply telling you what you did right or wrong. The key to reducing patient risk is through repetitive task training and self-reflection.

“Consistent team training can also improve performance and technical skills, ensuring the high standards of care delivery required by both hospital administrators and patients,” Singh adds. “Poor communication between multi-disciplinary teams is one of the leading causes of medical errors adversely affecting patient outcomes. This can even lead to lawsuits and escalating costs. Knowing how to work well as a team can be crucial to reducing medical errors and improving patient safety. There's no panacea for eliminating mistakes, but a starting point is clearly communication, which can be addressed through simulation.”

With malpractice insurance premiums continually rising, anything that can keep those costs down is crucial for healthcare facilities. This is one of the reasons behind the fast growth of medical simulation systems for training healthcare workers. “Malpractice insurance programs are already taking that into account,” Singh notes. “A hospital could benefit from a reduction in malpractice insurance because some of its employees participate in simulation training. That's a huge benefit for the hospital. Simulation training is a trend that is already taking shape in hospitals, and it will become more commonplace in the near future.”

EMS develops software that medical, nursing and allied health schools, hospitals and counseling programs use to run their clinical simulation training facilities efficiently. Background: There has long been a desire to close the well documented gap that exists for nursing students when transitioning from theory to a work setting. This gap is often described as a "continental divide" between nursing education and nursing practice (the Joint Commission, 2005). Nurse call lights are an example of a difficult task for nurses that are often neglected in traditional nursing school programs. Call lights require critical thinking, specifically in the areas of time management, prioritization, and urgency recognition. In order to meet this challenge, a nursing program integrated a nurse call into the current simulation case experiences to close the gap and strengthen students prior to entering the workplace.

Project outcomes: The purpose of this project was to systematically integrate a nurse call light system, both physically and theoretically, into a simulation program in order to help prepare student nurses for better adaption to the challenges of realistic nursing practice. Objectives which incorporated call lights were designed specifically for each nursing level. These objectives were then added into each of the SCEs along with the current subscribed objectives.

1) Describe the steps taken to integrate nurse call lights into simulation clinical experiences in order to develop ADN/BSN students in the specific areas of critical thinking, priority setting and urgency.
2) Review a systematic approach to physically install a nurse call light system in an Acute Care Hospital Simulation Center.

Scottsdale Healthcare Adds EMS’ SIMXpress for Military Medical Training

Scottsdale Healthcare’s Military Partnership will use Education Management Solutions’ (EMS) simulation management technology, SIMXpress, in its new Military Training Center located on the Scottsdale Healthcare Osborn Medical Center campus in Scottsdale, Arizona. EMS’ SIMXpress is a portable, wireless, web-based, audio-video recording, performance evaluation, and data management solution.

The 7,500-square-foot training center features high-tech human simulators that bleed and imitate battlefield trauma injuries to provide hands-on training to military medical personnel. With the addition SIMXpress, military medical personnel will be able to improve their clinical and technical skills and build their behavioral skills such as leadership, communication, and team work.

According to Vance Nesbitt, Scottsdale Healthcare Military Partnership Medical Simulations Coordinator, “SIMXpress is versatile enough to take out of the lab and into the field, enabling us to create more challenging and realistic field medical emergency scenarios. It will enable us to better record, view and evaluate military medical personnel during emergency scenarios.”
